As a patient, you have the right to:
- Access to Appropriate Care: Receive hospital services that are suitable for your illness or condition.
- Dignity and Confidentiality: Experience medical care with respect for your human dignity and complete confidentiality.
- Non-Discrimination: Receive service without any form of discrimination.
- Participation in Decisions: Be actively involved in decisions about your medical care.
- Healthy and Safe Environment: Expect a clean, healthy, and safe hospital environment.
- Named & Qualified Provider: Be treated by a clearly identified and qualified healthcare provider at all times.
- Voluntary Involvement in Teaching/Research: Participate in teaching and research activities voluntarily and under strict ethical standards.
- Privacy, Safety, and Security: Enjoy privacy, safety, and security within the physical limitations of your environment.
- Consent or Refuse Treatment: Consent to or refuse any diagnostic or therapeutic procedure. If you are unable to exercise this right, a close relative, caretaker, legal representative, or the consultant physician in charge will act on your behalf.
- Second Opinion: Seek or be referred for a second opinion, either within our hospital or from other medical centers.
- Continuity of Care: Experience uninterrupted medical care.
- Relevant Health Information: Receive medical and health information pertinent to your condition or illness.
- Receive Visitors: Have visitors when hospitalized, in accordance with our hospital's visitor policy.
- Redress Complaints: Lodge complaints, have them investigated by relevant bodies, and be informed of the outcome in a timely manner.
- Clinical Summary: Receive a clinical summary upon discharge or referral for seamless continuity of care.
- Make a Will: Exercise your right to leave a will.

- Previous Hospital attendance card
- Other relevant medical documents1 (e.g., imaging/lab studies, discharge summaries)

- Comply with Instructions: Follow instructions from hospital staff.
- Respect Rules and Regulations: Adhere to the hospital's rules and regulations.
- Remain Until Discharged: Stay in the hospital until formally discharged.
- Use Facilities Responsibly: Use hospital services and facilities with care and responsibility.
- Adhere to Treatment: Take prescribed medicines and adhere to treatment plans.
- Take Responsibility for Refusal: Accept responsibility for refusing investigations or prescribed treatments.
- Return Equipment: Return all hospital equipment/appliances upon discharge.
- Keep Appointments: Keep scheduled appointments and notify us of any changes to your address or contact number.
- Respect Others: Show respect and consideration for other patients and healthcare workers' rights, and help keep the hospital clean.
- Settle Bills: Settle hospital bills in accordance with hospital and government guidelines.
- Report Corruption: Avoid engaging hospital staff in corrupt practices and report all forms of corruption to hospital authorities or through the hospital's ethics and anti-corruption office.
